Description: Care Navigator
Salary: $35,000 per year/$16.83 hr.
Qualifications:
- High School diploma/GED required. Associates degree preferred
- Understanding of the concept and importance of whole-person care
- Strong communication and computer skills
- Ability to work effectively with consumers, families, healthcare providers and other community partners
Preferred Experience: Knowledge of community mental health and navigation of health care services. Work with individuals with severe mental health issues.
Duties and Responsibilities: Provide individualized, compassionate support in assisting consumers and families in understanding and navigating the healthcare system, available services, benefits, and community resources. Help consumers with appointment scheduling and following up on any barriers to accessing services. Assist consumers with connecting to their virtual behavioral health appointments in the office environment. Educate consumers on the need for follow up with primary care health concerns. The Care Navigator will work to facilitate a closed communication loop with internal and external service providers to ensure services align with individual needs, preferences and recovery goals.
The Care Navigator will also be cross trained for front desk fill in duties to be utilized as needed.
